1. Does E3 Possibilities, LLC take Insurance?

Currently we are a BCW  provider for Easter Seals North GA (Gwinnett County), GA Medicaid, Peachstate and Caresource Provider. We are out of network for all other insurances. We also accept private pay for therapy services at this time. We will provide a super bill for reimbursement purposes if needed. We will update this page if this changes. 2. How do I start the evaluation process?

Please contact 678.534.8137 or email adrienne@e3possibilities.com to request an initial evaluation intake packet which will include a consent to treat, request for referral, and appointment time request. Please contact your pediatrician to submit a referral via fax to (404) 445-5381. It is requested that ALL initial intake paperwork AND referral from pediatrician be received ATLEAST 48 hours before initial evaluation in order to allow appropriate time for clinical review.

3. Where will my child receive services at?

​

All services are provided in the child/family's preferred natural environment. This includes playgrounds, afterschool programs, daycares, churches, and home. Service locations should be within our driving radius and we should approve the location to provide therapy services before the initial evaluation.
